P Owen 6800 reported that he had seen Tom with a bad head and shoulder wound only being recognised by his papers, having been killed by a shell along with 2nd Lt V. B. Daniel, and  Pvt J. L. Slavin  M. M. and Pvt G. R. Henderson.  (Australian Red Cross)  He went on to say that all four were buried in a military grave, though others reported that they were just covered on the trench parapet.
